The Data Controller, in compliance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, hereinafter RGPD, informs you that the processing of your data will be to guarantee the registration and monitoring of the medical and health treatment provided at the centre; to ensure the continuum of care between the different health and social services; to provide the necessary information for the correct billing of the cost of the services provided, to complete your clinical history at the centre and to implement the mechanisms for the coordination of clinical histories that are developed.

We will also use your contact data for the purpose of sending you an appointment via SMS, based on the legitimate interest of the data controller in relation to the continuum of care and the provision of health services.

This information will be used by the administrative services and services directly linked to the health care of our entity, each within their own competencies, and may be sent in whole or in part to official public and private bodies which, for legal reasons or for reasons of material necessity, must have access to the data for the purposes of the correct provision of medical and health care, which is the purpose of the processing of this data.

The data provided will be kept in accordance with the health legislation in force at any given time, currently Law 21/2000 on the rights to health information and patient autonomy, and clinical documentation, which provides for a period of fifteen years from the date of discharge from each healthcare process in relation to the relevant documentation and a period of five years for the rest.

You are responsible for the veracity and correctness of the data you provide us with and you have the right to exercise your rights of access, rectification, deletion, limitation of processing, portability and opposition of your data in accordance with the provisions of the regulations on data protection. To exercise these rights, you must write to the Data Protection Delegate, at the e-mail address maortega@clinicasantacreu.com, and in any case you must attach a photocopy of your national identity card or equivalent.

Likewise, you are informed of your right, in the event that you do not agree with the processing carried out by our entity or consider that your rights have been violated, to lodge a complaint at any time with the Spanish Data Protection Agency.
